Yellow Dot can help you!

Beginning April 10, 2012, the Yellow Dot Program (YDP) will be available to WV Citizens of any age.  This FREE program is designed to assist people who have a medical condition or who suffer from allergic reactions to certain medications.  In the event of an automobile crash or medical emergency, the YDP is a “Golden Hour” cooperative of local law enforcement, E.M.S., fire and rescue, senior citizen groups, the WV Division of Motor Vehicles, and is made possible by the West Virginia Governor’s Highway Safety Program.

Signing up is easy.

All DMV regional offices and the Fairmont exam center offer YDP sign up at the information desk. Click on the location nearest to you for directions and hours.

If you choose to enroll you will be asked to complete the Yellow Dot Program Consent form.

Upon signing and returning the Consent form to your neasrest DMV regional office, you will receive a Yellow Dot Program folder, complete with a Yellow Dot sticker, and a Medical Information form that you, as the enrollee, must complete. DMV may not assist in its completion nor retain any information contained on the Medical Information form.

You will then have your picture taken and printed out. The printed picture is to be placed on the 
completed Medical Information form, the completed Medical Information form along with the participants picture is to be placed in the yellow dot folder provided and located in the glove compartment of the vehicle.  The yellow dot sticker is then positioned on the outside of the lower left rear window of the vehicle.
